ABOUT ADVANTAGE FLORIDA HEALTH PLAN |

- Independent Living Community Services, Inc. is a Public Non-Profit Entity type 501(c)(3) Public Charity doing business as ADVANTAGE FLORIDA HEALTH PLAN (“PLAN”). The PLAN is a PSN applicant which intends to offer certain health care benefits as a PSN to eligible Medicaid recipients pursuant to a Medicaid Contract with the Agency for Health Care Administration (“AHCA”) and in fulfillment of AHCA Grant. http://ahca.myflorida.com/Medicaid/psn/index.shtml
- PLAN will also provide administrative services to other Medicaid or Medicare Payers related to Provider Network Development and Management.
- Independent Living Community Services, Inc. purpose is promoting and providing supportive housing and home-and-community-based-services, public and provider education and on-going services enabling citizens to continue living in community based settings.
