** Will I lose my Medi-Pass patients?
ADVANTAGE FLORIDA HEALTH PLAN is open to Medicaid providers and supports continuity of PCP care as they align with enrollee choice. Your patients could be assigned to another PCP if you do NOT participate with a plan. Our goal is your participation.
** How will it benefit the PCP to participate?
This happens in several ways as follows:
- As stated above, we support continuity of PCP for Medi-Pass patients.
- $4.00 PMPM (Per Member Per Month) Primary Care Case Management Fee
- A cost savings program will be discussed with the PCP.
- PCP Medicaid rate of 100% while PSN is FFS. This increase will go to 110% when it becomes a Prepaid Capitated PSN or a Medicaid HMO is involved.
- PCP Medicare Special Needs Plan (SNP) rates negotiated at 110% of Medicare rates.
- ADVANTAGE FLORIDA HEALTH PLAN will have care coordinators that become care extenders for the PCP. This includes helping to identify potential specialists for your patient.
- Our social worker support helps keep the member Medicaid qualified.
- Our patient referral process helps keep the PCP from changing unnecessarily or setting up a file for what might have been a one time visit under Medi-Pass.
- Discounted monthly fees for Electronic Medical Record (EMR) and Patient Care Workflow Management System www.MDFlow.com . Express your interest in this EMR to your Provider Relations or Network Associate for more information. 25% monthly service discount for PSN participants goes to 50% for affinity PSN participants.
- We provide numerous management reports to our PCPs that identify opportunities for improvement in all healthcare categories. This includes support for Provider Quality Report Initiative (PQRI).
- We track utilization and share this information with PCP for input in quality cost effective care.
- We are facilitating a risk management program that would include a premium credit on your professional liability insurance.
- We are facilitating a program to assist PCP practice development and continuity at retirement.
You will have a voice through the PSN governing board representative. Please advise if you have an interest in serving on the board or as a part of PSN credentialing or peer review committees.
